Confidentiality & Privacy Policy

At Tapiz, we prioritize your privacy, safety, and trust. This document outlines how we manage your personal health information, the limits of confidentiality, and your rights as a client. We are committed to maintaining your privacy in compliance with legal and ethical standards, including the Health Insurance Portability and Accountability Act (HIPAA).

Confidentiality in Therapy

All communication between a client and therapist is confidential and protected by law. This includes records, session content, and personal information. Information will only be shared with your written consent, unless required by law under the following exceptions:

  • Suspected Abuse or Neglect:
    If we suspect abuse or neglect of a child, dependent adult, or elder, we are legally obligated to report this to the appropriate authorities immediately.

  • Serious Threats to Others:
    If a client makes a credible threat of serious bodily harm to another person or group, we must contact the authorities and may notify the potential victim(s).

  • Intent to Harm Oneself:
    If there is a serious risk of self-harm, we will make every effort to ensure the client’s safety, including involving emergency contacts or professionals. In cases where cooperation is not possible, we may take protective actions without consent, as permitted by law.

Confidentiality for Minors

Clients under the age of 18 have limited confidentiality under the law. However, we are committed to fostering trust and autonomy for young clients.

  • Children under 12: Parents/legal guardians may access most treatment information.

  • Adolescents aged 12–17: May be granted increased confidentiality during individual therapy sessions. However, exceptions apply in cases of danger, abuse, or when disclosure is legally required.

  • We encourage collaborative communication between minors and caregivers and will support this in developmentally appropriate ways.

HIPAA Notice of Privacy Practices

We comply fully with HIPAA, which governs how your Protected Health Information (PHI) is collected, used, and shared. You have the right to:

  • Access your records

  • Request corrections

  • Request a list of disclosures

  • Limit how your PHI is shared

  • Revoke your consent at any time (except when disclosure has already occurred)

Telehealth Privacy & Security

If you choose to receive services via telehealth, please note:

  • We use HIPAA-compliant platforms to ensure secure video and audio sessions.

  • We recommend clients participate in sessions in a private, quiet space and avoid public Wi-Fi.

  • We do not record sessions without explicit written consent.

Comunicación Digital y Almacenamiento de Registros

Ofrecemos comunicación digital para mayor comodidad y transparencia, pero existen límites importantes:

  • Correo electrónico y mensajes de texto: No son completamente seguros y se recomienda usarlos solo para fines administrativos o para agendar citas. Evite compartir información clínica o personal sensible por estos medios.

  • Registro de datos: Los registros de los clientes se almacenan de forma segura utilizando sistemas electrónicos de registros de salud (EHR) encriptados y compatibles con HIPAA.
